A big girl with a hidden hormone problem

Last week I saw a 32 year old woman who had battled with excess weight for many years. She was very frustrated – she had tried very hard to lose weight but it came off slowly and then she regained it all too quickly. She had seen a hormone specialist (endocrinologist) who had told her there was nothing wrong with her hormones and that she did not have Polycystic Ovarian Syndrome (PCOS) because her testosterone levels were not elevated and she did not suffer with acne and facial hair.

I begged to differ with the endocrinologist
; because this woman had the tell tale signs of PCOS in her pelvic ultrasound which showed excessive follicles lined up around the periphery of her ovaries. These were trapped eggs (follicles) that had failed to ovulate and if an egg does not ovulate it will not produce progesterone. Her history revealed that she did not menstruate regularly and she only had 5 to 7 menstrual cycles a year; this is because she did not produce the progesterone required to bring on a menstrual bleed.

She had a somewhat “Cushingoid appearance” with a very fat round face and a large fatty belly. Her blood cortisol levels were at the upper limit of normal and this is common in overweight younger women. All these hormonal imbalances were making her overweight and disrupting her metabolism and unless I fixed them she would have a life long battle with obesity and would become diabetic later on.

I prescribed natural progesterone cream to give her a regular menstrual cycle and stimulate weight loss. I also prescribed a low carbohydrate eating plan with lots of protein and raw vegetable salads to help her liver. I also prescribed supplements of magnesium and Glicemic Balance capsules to lower her high insulin levels, as this is vital to promote fat burning and weight loss.

Pro Fem cream contains natural progesterone and is rubbed into the skin once daily after a shower.

I am sure this woman will do well with her weight loss and her general health because it is quite possible to correct the hormonal imbalances she suffers with which are causing her excess weight – namely

Low progesterone due to PCOS
High cortisol levels
High insulin levels

Do you think you have a hidden or misdiagnosed hormonal imbalance causing your weight excess? If so, we'd love to read your comments.

  1. I suspect I have this issue and the progesterone solution is interesting. However, given my past history should I be concerned? I had a breast cancer scare and was on tamoxifen for 5 years to lower the estrogen levels in my body. Subsequently, I had a hysterectomy which included removal of ovaries. Given the breast cancer concerns, should I have concerns regarding introducing progesterone to my body? Thank you.

  3. The above described fits me to a tee. I have just finished yet another round of Provera* from the clinic, and i am already starting to bleed a thin flow with no sign of stopping.(as i am not producing an egg.)There is one difference with me however, unfortunately i have struggled with opiate addiction for 9 years, and am finally clean. I am sure however, that there is a great risidual build up complicating my things. Aside from my endocrine problem, what can i do to jumpstart the cleansing along with addressing my weight and menstration situation? Please help.

  4. Dr, Cabot, I KNOW that I have this issue and for years have not found ANYONE to listen to me. It has been a frustrating journey, NOT ONLY is my weight loss a problem, I have been misdiagnosed by the psychiatrict industry for bipolar disorder and have fought the diagnosis for years!

Now, as a result of all of the psych medications, I think I may have liver damage or disease. My hands are yellow my eyes are cloudy in color, but the doctor that I've been going to who kept telling me my liver enzymes WERE NORMAL, even though they were in the low range and I WAS CONCERNED ABOUT THEM, said it is not jaundice. I don't believe him, and need some help please! I have a young daughter who needs her mother back!

  10. I have been using a bioidentical progesterone cream Oasis Serene for several months and am getting great symptom relief. My doc recommended it for hormone imbalance. I am also using a very good herbal product to cleanse and support my liver, LiverPure. Also, I have changed my diet using metabolic typing. Every day I am feeling better and better. Thanks Dr. Cabot for all the information you give about the importance of cleansing and supporting the liver!

    Dear Dr. Cabot:

I am 52, started BHRT in Fall 2008, and was on top of the world until this last Dec. I have been diagnosed with biliary dyskinesia, but ultrasound showed no stones (I had done a liver flush prior, though). Since Dec, I have gone from 130 to 114 pounds, with ALT and AST levels normal (but high MCV and MCH, and I'm short of breath all the time except after I eat). Already following your diet and with supplements suggested, but my right side (sometimes moving across to the left side) is chronically cranky. I have been researching estradiol's role in all this, and dominance seems the culprit, but I have also seen studies that progesterone causes hypomotility of the GB. And it seems I get a lot more bile reflux when I'm on my progesterone cyle. I saw a surgeon yesterday for the first time to discuss removal, but I am scared of this step. I know it's my hormones out of whack, but don't know what to do (my BHRT doctor doesn't seem to know either). ANY HELP OR KNOWLEDGE WOULD BE INVALUABLE - THANKS!

  13. Dear Eugenie

Make sure you are not vitamin D deficient; that is the most common cause of osteopenia. On a blood test your vitamin D level should be at least 42ng/mL.
Celiac disease can cause osteopenia and infertility.

We recommend a low carbohydrate diet as the most effective and healthy way to lose weight. See Dr Cabot's book "Can't Lose Weight? Unlock the Secrets that keep you Fat".

  14. Dear Rebecca Miller

I am 52, started BHRT in Fall 2008, and was on top of the world until this last Dec.
THE CREAMS CONTAINING BHRT ARE SAFER THAN THE TROCHES AND PLACE LESS STRESS ON THE LIVER AND GALL BLADDER

PLEASE CHECK YOUR SERUM VITAMIN B 12, VITAMIN D AND SERUM IRON STUDIES

Already following your diet and with supplements suggested, but my right side (sometimes moving across to the left side) is chronically cranky.
MAGNESIUM SUPPLEMENTS AND SELENIUM CAN HELP TO REDUCE BILIARY DYSKINESIA

I have been researching estradiol's role in all this, and dominance seems the culprit, but I have also seen studies that progesterone causes hypomotility of the GB.
    CORRECT
BUT CREAMS ARE SAFER THAN TROCHES AND ARE LESS LIKELY TO STRESS THE LIVER AND BILE DUCTS

And it seems I get a lot more bile reflux when I'm on my progesterone cyle. I saw a surgeon yesterday for the first time to discuss removal, but I am scared of this step.
    IT IS A LAST RESORT
CONTINUE WITH LIVATONE PLUS AND RAW JUICING
USE LOW DOSE BHRT IN CREAMS, DROP THE TROCHES

How can I get access to these products? I tried to communicate with the North American Website Contact page, but have been unsuccessful, as it won't upload my request.

  17. Hello there

After buying the Liver Cleansing Diet book by Dr Cabot I am now into the third week of the diet plan.

Can you please explain why it says in the book you can't eat eat yolks in the middle four weeks but you can eat pasta dishes which are made with egg yolk?

Also, can you please provide further information on the biological process the body goes through during stages 1,2,3. Is there a reason for it being 8 weeks long? I am keen to understand the process further.

Also, do you know how many supplements a day the body can handle? I am taking Livatone Plus with numerous other relevant supplements each day. Is this okay?

    Thanks Nicola Lumley

Dear Dr. Cabot, it is Natasha again..you will note my comments from above. I have been on Livatone Plus for 2 weeks now but on the low dose of one capsule a day. My ANA blood levels are positive again, my skin rashes have increased, my dizzy spells have come back, I have lower back discomfort, darker urine then before, tend to have mucous and have developed a cough. What is going on...please help. I had also emailed as per Margaret's note above - but have not heard back since. Am I getting worse?

  20. Hi Doc,
Am Kofi from Ghana. My sister has been battling with what is believed to be "cushings syndrome" for 13 years. Nothing seem to work. For me i strongly believe something went wrong with diagnosis as reading from your website and comments from other people help me to think my sister might be suffering from hormonal imbalance, may be thyroid problem or liver problem or a combination of all.

What I am happy about and very certain is that, you can fix her. Could you please provide an email so I can send you details of her to you including pictures....?

I know you can help my sister get well again, Please do, for you are our last resort, Doc
Thank you and looking forward to hear from you. urgently

I have fatty liver and I would like to know if Chardon de Marie is really delivering what they said. Excuse me but on line selling is really expensive, and do not know how reliable this product is. do you know where to get this product not too expensive?

Hi, my name is Froso, I'm from Cyprus
I found your book for syndrome X very interesting!
I think I may have the same problems as the woman above, I've been diagnosed with metabolic syndrome X 4 years ago and I found out I have polycistics a few months ago. Doctors are not specialized in syndrome X over here and I am very disapointed because I cant find the help I need. I'm struggling with my weight since I was 10 years old, now I'm 29 and I need to do something drastic!!! I found out from an ultrasound that I did because I had terrible pains in my stomach area that I have also a fatty liver. I read your book and I want to do the 6 week liver detox diet, but I have a few questions, for instance you dont mention if we are allowed to eat any dairy products during that time, So for the long comment I didnt know any other way to contact you, since I'm far away!!! Thank you FROSO
